2. Complete Your Health History Form (VERY IMPORTANT!)
Please set aside at least one hour to fill out your Health History Form as completely as possible. This form is an essential part of the process, as it gives us deep insight into your health, lifestyle, and risk factors.
Deadline: Complete at least 3 days before your appointment so that we have enough time to thoroughly review your responses and tailor your consultation to YOUR unique needs.
3. Come Prepared – This Is About YOU!

- A list of any current supplements or medications you’re taking
